Holes in faces

Currently, the triangulator/importer does not support holes in faces unless you get fancy with how you apply facet creases to force it to work. For example, this will import correctly (with a square hole):
In general, this is related to an issue in FOLD where faces do not support holes, see Issue edemaine/fold#21
