CMS data hard coding issues

Two issues emerged in assembled the NADP demo. Blue-button parsing of CMS data needs to be hard coded to 1) Have demographics address coded to 'United States' instead of 'US', 2) Phone number needs to be 'Primary Home' instead of 'Primary'
