Investigate if VS Code window needs to be reloaded after installation

Description

Currently sometimes the plugin will fail to register post installation and display such a message when run:

Acceptance criteria

  • The extension should work out of the box without a reload
  • If a IDE reload is needed, the user should be alerted to do so.

Looks like this is no longer an issue with the newer version of VS Code https://code.visualstudio.com/updates/v1_31#_no-reload-on-install

I'll leave this issue open in case people run into it in the future.
