Add feature to report one-time-use code leaks

In attempting to convince other jurors to vote the same way, a juror may leak their one-time-use code to show how they voted in a given round of a dispute. To dis-incentivize this, Aragon Court has a mechanism to penalize jurors who leak their code. However there is not currently a feature in the Aragon Court Dashboard to trigger this penalty.

I would like to see a feature in the dashboard that makes it easy to report one-time-use code leaks and trigger the penalty against cheating jurors.
