Duplicate request fields are not editable

As a researcher,

I want to be able duplicate a previous request's information into a new request and make tweaks to it

so that I can quickly create my new request without having to enter the bulk of information that was entered previously.

TESTCASE

On a approved request page, click "Duplicate Request". On the new request page, click on a field other than request title.

EXPECTED

Field is editable.

ACTUAL

Field is not editable. Request title is editable, but the rest of the fields are not. However, if you edit the request title field then you can edit the other fields.
