Changelog is behind hex.pm versions

Changelog latest version is 2.2.0, hex.pm has 2.3.1.
For PR contributions, is might help adding a .github/PULL_REQUEST_TEMPLATE.md asking people to keep the file updated.

Right - I'll hopefully get some time soon to add to the changelog.
