Add new issue/PR template

This is a small improvement so users would have a clearer view of what we expect to have in the issue/PR:

Templates could contain:
- Package version (in case of issue report)
- Python/Django version (in case of issue report)
- Snippet to reproduce the issue (in case of issue report)
- Expected and actual behavior (in case of issue report)
- Should be considered as a breaking change or not ((in case of PR report)
