Error executing Server Side Ad insertion example

I'm trying to execute the server side ad insertion example but I'm getting an error. Everything seems to be working fine until the HLS manifest creation. The error text is Segment for Custom Tag not found!. tried several times with the javascript sdk and the dotnet sdk, always get the same error. Any help would be appreciated.
