add license information and related notices

Please add a license. For simplicity we could use the same license used by the source material.

This should pay attention to other stipulations in the source license, e.g., regarding notification of what content was adapted and what changes were introduced (this would be limited to the one notebook that was adapted, so should be straightforward).
