flash drive not seen by file explorer, disk management and disk partition.

i recently formatted a four GB flash drive and it hard been corrupting my files. but after formatting, it started showing some errors and it prompted a fix disk procedure. i did just as recommended but the errors could not be fixed. i cancelled and unplugged the device then plugged it back into the usb port and transferred some files to the disk. i opened the files via the disk which after a few minutes disconnected itself without unplugging it. i tried all web based solutions but nothing has so far worked out. can anyone please help me?
