Make registered authenticators show on login page

Every login method should do one of the following:

• A single button login that redirects elsewhere (including away from the application in the case of OAuth)
• A button that shows a single form with additional required info (username/password, email/password, phone/password)
• A button to shows a smaller form asking for the user ID and submission, then a new form requiring a one time password.
