Source is missing in "Report this content" flow

Description
After a user goes through the "Report this content" flow, we display a message encouraging them to report the content at the content's source. Currently, that source is blank. This was not the case when this feature was launched, so this is a bug that has been introduced since.
Reproduction
- Go to a single result page, e.g. https://search.creativecommons.org/photos/82be00eb-9d9d-4778-9743-df3d553cac28
- Click on "Report this content" and choose _any_ of the three options (Copyright, Mature, Other) and complete the flow
- The problem is apparent. There is a period in place of where the source name should be.
