Example 4.5 on page 12 is missing a definition of j.
I assume it's from an inner loop that had gone AWOL..

Could you download the latest version and see if this is still missing? I think this is something someone else mentioned yesterday that's been fixed in e86b766 :).

I'm going to close this to clean things up. Let me know if the inner loop you're referring to is still missing - I think this has been fixed. :)
