Update CyHy reports and daily notifications to include associated domain name

Summary

Adjacent to the CyHy domain name scanning functionality MVP planned to be rolled out in via: Modify existing CyHy system to include MVP domain scanning capability #70; this ticket is tracking the report and notifications changes. Mock ups have been provided by the business owner for reference.

Acceptance criteria

• Include the domains, the IP address each resolved to, and the association between them in the customer report.
• Add columns for the domain and IP to report attachments.
• Add columns for the domain and IP to vulnerability notifications.
• Add columns in scope.csv, hosts.csv, findings.csv, etc.
• Report reason for closure in mitigated_findings.csv (examples: port not open, ip changed, vulnerability not detected, etc.)
