"Save" button for self-hosted not currently enabled

Expected behavior: After data is filled in, "Save" button goes live to save

Actual behavior: Save button does not "light up" to enable a save

Steps to reproduce:

Go to self-hosted tab of browser extension. Fill in data. Try to save.

Urgency:

Blocking some large self-hosted customers from using browser extension

The user must click the checkbox in the "Codecov URL" field and grant permission to enable the Save button

I think the UX might be a source of confusion here, made a PR to address this
