Velocity command not executed by all wheels

Occasionally (trigger conditions not yet clear), one wheel (usually stern) will continue to move when the other two stop. I have seen this happen during a pause and during a commanded zero velocity. It may happen for non-zero velocities also, this would be harder to observe and I have not looked for it.
