Add Issue template to the project

i've been looking at the issue and i believe that introducing standardized issue templates could help ensure we gather all the necessary information right from the start.

• bug report
• feature request
• etc

could be really beneficial.

this is just an idea, but I think it would help make our issues clearer and enhance our productivity.
what does everyone think?
