Notes not always triggered

Sometimes a note is not triggered on start playback. E.g. the keyjazz in instrument macro editor.

Also the first note on startup may cause noise + when stopping some channels can make sound.

This all probably has something to do with keyon/off flipping.

Seems to be fixed now.
Replayer now makes sure to have a full on-off-on cycle.
