History table only shows placeholders for the head row

When searching for a user, the table displayed doesn't show the correct headings.

Instead of this (as shown on all other pages):

It looks like this:

This issue is also present on the demo version of the website, so I created this issue in an effort to help improve the base code.

I don't know much about PHP, so I was hoping someone more skilled could help out.

I'm just now seeing that the code on Spigot, that links to this repository, looks completely different. Therefore, this issue might not apply here.
