Improve monitoring and logging

We really need detailed logging information for server errors and latency issues and improve the monitoring report. Some concrete areas to investigate:

• Make it easy to get to the logging/error page from Alert email.
• Make handbook for each alert.
• Log the request level information for any monitoring errors or latency issues. Ideally, we would like to log the request when latency is above certain threshold.
