Line/No Line Class for form headers

On 3/17/17 we discussed a scenario where a form has multiple sections and thus made sense to have a visual separation between sections.

In the style guide we need to add the ability to have a form with no lines (forms with 1 section) and forms with lines (forms with multiple sections).
