I am stuck on download manager 4.3.2 and there seems no way to add updates to the download sites the edit and apply do not alter any of the list. I am able to only enable and disable. Any thoughts and suggestions?

Don't you have an "Add" function in your download manager's settings?
Alternatively, according to the Addon Developer's Guide you can drop your plugin(s) in /share/MD0_DATA/.qpkg/DSv3/usr/sbin/addons/. This path is valid for DM 5.1, it may be different on your system.
