SPEC: Update `_app` and `_scope` to `mp_app` and `mp_scope`

To better identify that the _app tag (provided if a mp.metrics.appName mp config value is set) and _scope as tags that originate from MP Metrics, will prefix it with mp to better represent that it originates from the MP Metrics runtime and to better avoid any potential name collisions.
