Cannot reproduce with VintageFix 0.3.1, and Extra Utilities 2 1.9.9. All blocks and items in the creative menu render without issues.
Your log mentions "VintageFix 1.2.4", a version that does not exist to my knowledge (all official versions I have released were 0.x). Additionally, you are somehow running FoamFix at the same time, which should be impossible, as I have marked VintageFix as incompatible with it.
It therefore seems likely you are using a modified version. Please verify that the issue can be reproduced in the official beta or release versions. I cannot provide free support for unofficial forks.
