Ron Jeffries proposes a three step process that I like to use:

1. Card

The Issue itself

Brief description to understand the who, the what and the why

As a kind of user,
I want some objective
so that some reason (outcome).

2. Conversation

Comments happening in the Issue, that ideally provide executable examples.

3. Confirmation

Acceptance Criteria that test if the Story has been implemented correctly
