Rename to docker-machine-driver-equinix-metal

Rename this project (internally and externally) to docker-machine-driver-equinix-metal.
Should the org name also be updated in this rename? (Docker machine driver URLs are embedded in projects like https://github.com/packethost/ui-driver-packet (see equinix/ui-driver-metal#22).
The org name should also be updated.
The ideal target should be equinix/docker-machine-driver-equinix-metal.

ui-driver-packet references these release assets by URL at: https://github.com/packethost/ui-driver-packet/blob/master/README.md#using-the-equinix-metal-ui-driver
These URL references (those included in existing Rancher documentation and installs) will continue to work after we move this project to https://github.com/equinix/docker-machine-driver-metal.
