footnote crash

Describe the bug
When running mdformat on the following Markdown, it crashes.
[^a]: Lorem. [^b]
[^b]: Ipsum.
A [^b]Not saying the above makes a lot of sense, but mdformat should probably not fail hard.

Moved to mdformat-footnote as this doesn't reproduce without the plugin.
