Add continuous app location + size tracking

App windows may move or resize, outside of WTQ's influence, for example when attaching or detaching a monitor, or changing resolutions.

It'd be nice if apps are automatically reorganized in these situations.

Currently it can be worked around by just toggling all apps in- and out, but it's a bit annoying.
