Create new donation form on Neon

This should be pretty straightforward with some good judgement, one thing to remember is we want to send a source field, probably, so that the form could be at a link like, say:

https://donate.free.law/?source=more-alerts

Or whatever. Something like that.

For now, this will be about individual members, but v2 will be about organizational membership too.
