Create a custom tool for NDVI Statistics

NDVI Statistics should display a custom form for running.

NOTE: this is enough generic to allow the creation of a widget that gets a form with the fields to use from the general configuration. We should create this widget on the MapStore master branch and merge it for our purposes.
