Update documentation with CSV null value handling

Hi,

Creating an issue to track a documentation change - will submit a PR shortly with proposed additions.

This is to clarify how null is handled in CSVs, and how to modify the null token. (Please let me know if this is already documented somewhere else, was thinking of adding notes to db-project-structure.md.)
