Add a doc about Error Conditions and Reporting in doc/spec

Problem
From the discussion of #1947 (comment), it would be useful to add an Error Conditions and Reporting doc in https://github.com/google/knative-gcp/tree/master/docs/spec.
We can put information about error signalling https://knative.dev/docs/serving/spec/knative-api-specification-1.0/#error-signalling and examples we used to standardize condition marking (like knative/eventing#2187 (comment)) in the doc. It would be a reference for engineers to handle issue related to marking conditions.
