Widget will not perform transaction

Does this widget require an ArcGIS Online Portal account? I cannot seem to get it to work.

GP service it uses is wide open:

I did see the rest service. I must have installed it incorrectly. When I click the line to start the profile nothing happens. I will retry. thanks

I had to add 'div class="gis_DrawDijit" in order to get the icons to show in the widgets. I am using an https version of the elevation service. The mapClickMode is set to true. I cannot get past click. It seems to disable click for the Identify widget as well. Any thoughts?
