Add delete entry buttom to Per-site permission list

The Per-site permission has the option to add a site/URL. But it hasn't a option to delete this entries.

Setting any entry to DEFAULT actually deletes it (you won't see it anymore in the list if you refresh it).

@hackademix That is a very difficult and no intuitive mechanic.
