When defining a new code without evidence it can be created without name

Steps to reproduce

Go to codebook development mode. Click on "Create new code". It will ask you to create an empty code, click yes. Then don't set any name, leave it empty and click OK.
It should check before creating the new code that the name is empty but it is not

How to solve

Check before creating the annotation that the code name is not empty string.
