The Add Bio-Data modal shows incorrect text

The Add Bio-Data modal shows incorrect text when systolic, diastolic, or triglycerides are selected. Currently the text reads "Note: accounts for of your overall health score”. Instead the text should be a full sentence indicating percentage of health score.
