ActiveParameter set to null after switching programs

What steps will reproduce the problem?
I'm not exactly sure what steps reproduce this issue but it usually happens
after saving a new program from the plugin and then some combination of
switching the active program from the host and/or from the plugin.
What is the actual output?
ActiveParameter for every VstParameterManager in VstParameters is set to null
and causes an exception.
