Add the new #{date} place holder to bpmn and fhir files, switch to #{...}

We should add the new #{date} place holder to fhir files and switch to the new #{...} pattern for place holders introduced by highmed/highmed-dsf#248
Our bpmn files should use the new place holder substitution mechanism and use #{version} instead of hard coded values.
