Terminal pwd changes

At present, the pwd on initial display of the terminal is the ICEcoder dir as it's based on the location of terminal.php. However, when you run a command, the pwd if that of lib/terminal-xhr.php and so the pwd is of ICEcoders lib dir.

Need to make these not only consistent but also set initially to the doc root.

Fixed and also fixed issues with cwd too.
