Poor MAM performance with large archives (2.2.0)

Version 2.2.0 introduces a problem where MAM queries take a very long time to complete in systems that have message archives that contain many messages: We're seeing multiple-minute response times for MUC archives that contain 70,000 messages.
In 2.2.0, different database queries are introduced. It is likely that the existing database indexes no longer suffice.
