Inadequate Indexing Documentation

We need better information on Indexing in the documentation (Perhaps as its own
page). Specifically, there could be more information on (1) the different types
of indexing that we allow and (2) when and where indices will actually be used.
We also need updates on the latest changes to indexing. I would be glad to be
the author or co-author of this document, as Ildar and I have made a bunch of
the changes, but we need someway to collect the knowledge from everyone who can
contribute.

Actually our branch modifies documentation and gives couple of examples of how
open\nested indexes should be created.

These changes will cover our case but I think that (1) and more specifically
(2) are not addressed in general.
