Forms should support additional custom fields

Description
The form builder currently only support a text field as a custom field option. This should include textarea, select, and radio.
Acceptance Criteria
- Textarea custom field is available in the form builder.
- Textarea custom fields render in the form template.
- Select custom field is available in the form builder.
- Select custom fields render in the form template.
- Radio custom field is available in the form builder.
- Radio custom fields render in the form template.
