Need to add the fields extracted from the demographic survey into the profile form when it is sent by Dr. Sherri

Add "Years of practice since completion of training"
Less than 5 years; 5 - 10 years; 11 - 15 years; 16 - 20 years; More than 20 years

Add "have you ever had virtual reality or online training? - checkbox
follow up if checked - Add "Which virtual reality or online training have you been exposed to?"

This is completed. There is a mHBS folder in Box which has screen shots of the final Profile form for the mHBS/eHBB tool. It is de-identified (e.g., no fields for name, etc), for purposes of the research study. The Enrollment Form for the mHBS scale-up tool will have a more flexible format.
