I'm using the latest tt-rss version and installed your plugin. There are no errors when activating the plugin, but there is also no icon in the menubar. Maybe you can look into compatbility:

I'm using the version v1.11 as well with no issue.
No icon should apear in th ction menu, but you should have one at the bottom of th article (which is not visible in you screenshots :) )
Could you check your server logs for errors ?
Have you trid tt-rss 1.13 ? (full disclo : I did not, I don't know if th plugin will still work)

Tested with version v1.15.3.e6c826f and it works.
