Do you know how to use this index https://docs.djangoproject.com/en/2.0/ref/contrib/postgres/indexes/#gistindex
instead of migrations (https://github.com/jneight/django-earthdistance#for-django--17) ?

Its not good beacuse extensions could not be loaded before.
