Hello!
At first thank you for share that script!
I wonder how could be possible to use radio and checkbox type?
here is my sample of checkbox which is unable to Post:

as i can see in my form there is the same entry id "entry.254239171" only value is changes Once i POST noting is appear in my answers. Thank yoy
