Addon builder doesn't applied

Hey all,
I have tried to create addon, library loads and displays template for it, but does not apply custom builder function (passed as 4th argument to schemaFormDecoratorsProvider.defineAddOn)
I have looked for example here
What am I missing? Any thoughts?
