In fact, the whole subdomain mongly.openmymind.net is dead for me -

Not just you. I killed the server it ran on a while ago.
At some point I might get around to updating the book, but I'm not sure I can even re-generate the pdf right now without some hassle so, ..it's kind of a low priority.
