Issue triage process

Should there be an issue triage process? E.g., do all design proposals come to the TSC, or do the maintainers of each repo have their own issue triage process?
Are issues discussed in a meeting or merely in github? Or slack?

I think a Github issue should be opened for a design proposal and then it can be discussed in the TSC for feedback, acceptance, or rejection. Design changes can have impacts across repos, so I think these type of changes make sense to have TSC level visibility and guidance.
