Whenever I upload a file, the upload completes at 100%, but then the page is refreshed/redirected, and loads the same page over again "admin.php?page=wp-downloadmanager/download-add.php" but I receive my sites 404 page.

Sorry man, I just tried this on my wordpress dev install, I can't reproduce this. Feel free to reopen it if you can reproduce with a fresh install of WP with the plugin.
